Family Connects Durham

Family Connects Durham is a free early postpartum nurse home visiting program that offers trained nurses to support parents and their newborn in Durham County either in-person or virtually. Families can expect to receive between 1-3 visits. The nurse can provide a health check for birth parent and baby, brief lactation counseling, answer questions a family may have about postpartum recovery or their new baby, help make an appointment for the 6-week OB checkup, and make referrals to additional community resources that parents may need. This could be connections to mental health services, nutrition or health insurance (SNAP, Medicaid), childcare, early childhood and parenting services (including longer term home visiting programs), as well as pediatricians and pediatric dentists. Family Connects Durham nurses can help connect a family with the other services described in this Resource Guide.

Who We Serve:

Any family with a newborn baby (0-3 months) who lives in Durham County

Nurses visit families in their homes; telehealth visits are also available as needed
